LEMDO: Schematron, ticket triage, fixes

Went through a couple of new GitHub tickets which appeared to be bug reports for me, but were actually problems caused by encoding issues. Then added Schematron to catch non-sequential paragraph ids, but had to comment it out because it caught too much; wrote some XSLT to renumber paragraphs, and if I have the OK from JJ I’ll run that on the broken files and then enable the Schematron.
Other tickets were a bit vague to act on, or don’t appear to be good ideas on the face of it.
