Switched the MoEML project over to using meta[@charset]

Following the model of other projects, made this switch and tested as far as possible. The build process on Jenkins will show any fallout I haven’t caught. There are a lot of places in MoEML where HTML head elements are being created. I haven’t looked closely at what we should do in this regard with the epub stuff, because that isn’t being published yet, but we’ll probably have to address it at some stage.
