Reorganization, refactoring, and some basic decisions

Renamed some XSLT to allow for easier grouping and management; updated processing to handle this; switched out the older Keats-derived template model for the one I use in HCMC Journal (a single HTML page template); raised a number of issues around the project title, title page, layout and other things with KB, and had some discussion by email.
