Following much discussion with John, we need to split the current TOC page into an Articles page and a Reviews page. Reviews need to be sorted on some basis that isn't quite clear yet; articles need to be title-sorted without their initial articles, which might not be so easy to do in XSLT, so for both cases it seems we might require a <title type="sort">.
