Thanks! I just talked to Ray, and he had some more comments/changes to the table of contents:
1 - Remove the View column, in favour of...
2 - The title of the article should click through to the XHTML version.
3 - Can we add two icons after the title, linked to the XML and TXT versions?
4 - The Vol/Issue column can simply read Issue.

Would it be possible to disambiguate (by class) hrefs, internal links, and notes? Right now, they are all classed as RefLink.

I spoke with Ray about this problem, and he decided to leave the hrefs as they are. At the time of publication, the reasoning was that the article should be able to stand on its own, even if printed. Revising the XML manually is too labour intensive.
